Referral Guidelines
All GP referrals should be made via eRS.
Consent
Form for adults who are unable to consent to investigation or treatment
(Consent form 4)
PEG Referrals
When referring for PEG placement, PEG change, or PEG removal kindly send a referral to the Gastroenterology department including the following information to ensure a timely and effective appointment can be made:
- Medical history
- Respiratory function, including SaO2 on air lying flat
- Mobility, if not independent please give details
- Communication difficulties, if so please give details
- Anticoagulant or anti-platelets agents, and other medications
- Antibiotic allergies
- Ability to consent, presence or absence of capacity. If lacks capacity or can't consent please give details of best interest discussions, supply relevant documentation, and include a 'consent form 4' - see above - completed by the requesting community team - e.g. the GP. Information to aid discussion is on the endoscopy page
- If communication is limited, or capacity lacking kindly ensure the next-of-kin attend with the patient
- If the PEG is to be changed or removed, please confirm the tube advances and rotates easily in the stoma
- If the PEG is to be inserted, kindly send an MRSA screen (nose, groin) and FBC and INR in advance
