Hospital Passport

The Hospital Passport is to provide our staff with information about your child during a hospital visit. This information will help us to meet your child's needs when using our hospital services.

The Hospital Passport uses a traffic light system. The RED section if for things we must know about you. The AMBER sections is for things that are really important to you. The GREEN section is for things that you like and dislike.

Please bring the Hospital Passport with you if you have to go into hospital
