Fit Notes

After you have left hospital or been seen in outpatients you may need further support.

For example, our teams (rather than your GP) should provide you with a FIT note, where needed.

If you're unwell and cannot work, you may need a fit note (sometimes called a sick note) for your employer.

If you did not receive a FIT note or for other queries we encourage you to contact the department whose care you are or were under.
